What to Expect From Your First Therapy Session
Starting therapy can feel like a big step. If you have never had therapy before, you might be wondering: What will happen? What will I have to talk about? Will I be judged?
Why is it normal to feel nervous before therapy?
It is completely normal to feel nervous before your first therapy session. Many people feel a mixture of curiosity, hope, and uncertainty when they first reach out.
Some people decide to try therapy after hearing positive experiences from friends or family. Others may have grown up with the idea that therapy is something to hide or feel embarrassed about. Although attitudes towards mental health are changing in the UK, there can still be stigma around asking for support.
The truth is, reaching out for therapy is a meaningful step towards understanding yourself and your wellbeing.

Your first therapy session is a space for you
Your first session is a relaxed and confidential space where we can explore what has brought you to therapy and what you would like support with.
You may already know what you want to work on, such as anxiety, stress, burnout, low self-esteem, relationships, or feeling overwhelmed. Or you may simply feel that something doesn't feel right and you would like a space to understand yourself better.
Sometimes, what people need most is simply to feel heard.
In our busy lives, where we are often rushing from one responsibility to another, having someone truly listen without judgement can be a powerful experience.
If you have taken the step to contact a therapist, it is already a brave decision. You don't have to wait until things become unbearable before asking for support.
What happens during the first therapy session?
The first session usually involves an assessment. This helps your therapist understand your experiences, your current situation, and how they can best support you.
You may be asked some questions about different areas of your life, including:
- What has brought you to therapy at this point in your life
- How you have been feeling emotionally
- Your relationships and support network
- Your childhood and upbringing
- Your work and daily life
- Your interests and things that are important to you
Understanding your background helps your therapist gain a fuller picture of who you are and what may be contributing to your current struggles.
You may also be asked to complete a questionnaire about how you have been feeling. Some questions may include your physical health, any medication you take, or whether you have experienced thoughts of harming yourself.
These questions are a normal part of safe and ethical therapy practice. They are not asked to judge you, but to make sure you are supported in the right way.
Is everything I say in therapy confidential?
Yes, therapy is a confidential space. What you share with your therapist stays private, with the exception of situations where there is a serious concern about your safety or the safety of someone else.
Your therapist will explain confidentiality during your first session, so you understand how it works before you begin.
What happens after the first session?
At the end of your first session, you will usually discuss practical arrangements, such as how often you would like to meet, booking future sessions, payment, and cancellation policies.
Your therapist will explain what to expect moving forward, including how much notice is needed if you need to cancel, reschedule, or take a break from therapy.
You don't need to have everything figured out
Many people worry that they need to know exactly what to say in their first therapy session. You don't.
You don't need to arrive with a perfectly organised explanation of your feelings or your life story. Therapy is a process, and your therapist will support you in exploring things at your own pace.
Starting therapy is an opportunity to pause, understand yourself more deeply, and begin creating meaningful change.
